What Is the Host Home Model?
The Host Home model places adults with intellectual and developmental disabilities in the private home of a trained, compassionate provider — someone who opens their home and becomes a genuine support in that person's life.
Unlike group homes or institutional settings, Host Home placements are small, personal, and community-integrated. Individuals participate in household routines, build relationships, and live with dignity in a real neighborhood.
At Acuity Meadows, every Host Home placement will be backed by nurse-led clinical oversight — so case managers can refer with confidence, knowing health and safety are never an afterthought.
